Family Support Program Eligibility
Eligibility criteria for Children is as follows:
- the family member/s have a disability as defined under the Queensland Disability Services Act 2006
- the family member/s with a disability are under 18 years of age
- the family member/s with a disability must be living with their birth or adoptive family
Process for determining eligibility and priority of need
All applications for the Family Support Program are assessed against predetermined priority indicators as set out in the Disability Service Qld Agreement. This information is contained in either the Disability Services Qld application form from the DSQ web site or you can download the application form . A Lifeline Panel is convened when a vacancy is available to determine priority of applications to hand.
Clarification
Following assessment, families will be contacted by the Family Support Program staff member to substantiate the information provided in the application form and to confirm the level of support required as appropriate for their needs. Eligibility for the Program does not guarantee that each family is able to be supported.
Review and Reassessment
Supports provided through the Family Support Program are on an as-needed basis, dependent on assessment of need and funding available. All supports provided through the Program are subject to ongoing review, to determine the support needs for the person with a disability and their family over time.
The Programme runs for a three year period with each family who is in the Family Support Program required to be part to a tri annual external assessment.
A process for program exit and transition may also be undertaken for families who no longer require ongoing program supports or who are no longer eligible for the Program.
One-Off Non-recurrent support
The Family Support Program provides one-off non-recurrent supports (when funding is available) to assist families with one-off supports/aids/equipment or services.
Eligibility is the same as for the Family Support Program.
The Process of notification of this funding availability is, but not limited to, newspaper articles, T.V., mail outs to the broader community, Community agencies, Schools, Hospitals and specific disability agencies i.e. Cerebral Palsy Association.
